摘要:
目的:探讨胰腺神经内分泌肿瘤(pNETs)的临床病理特征、诊治经验及预后因素。方法:回顾性分析157例经病理明确诊断的pNETs患者临床资料,其中胰岛素瘤83例、非功能性胰腺神经内分泌肿瘤(NF-pNETs)74例。结果:腹部超声、增强CT、增强MRI和PET-CT对pNETs的检出率分别为42.7%、87.7%、95.8%和100.0%。CgA、Syn、CD56和NSE阳性率分别为65.9%、99.4%、72.7%和98.0%。NF-pNETs与胰岛素瘤相比,肿瘤直径及Ki-67指数偏大,WHO分级偏高,AJCC分期偏晚(P<0.05)。发病年龄、肿瘤有无功能、WHO分级、AJCC分期和有无淋巴结转移与pNETs预后有关(P<0.05)。结论:影像学及免疫组化检查有助于pNETs早期和鉴别诊断。相比胰岛素瘤,NF-pNETs恶性程度较高,预后较差。
Abstract:
Aim:To investigate the clinicopathological characteristics, experience of diagnosis, treatment and prognostic factors in patients with pancreatic neuroendocrine tumors(pNETs).Methods:Clinical data of 157 patients who were pathologically diagnosed as pNETs were analyzed retrospectively. Among them,83 with insulinoma,and 74 with non-functional(NF)-pNETs.Results:The sensitivity by abdominal ultrasound, CT, MRI and PET-CT were 42.7%, 87.7%, 95.8%, and 100.0%, respectively. The positive rates of CgA, Syn, CD56 and NSE were 65.9%, 99.4%, 72.7% and 98.0%, respectively. Compared with insulinoma, NF-pNETs had larger tumor size and Ki-67 index, higher WHO grade, and more advanced AJCC staging(P<0.05). Age of diagnosis, tumor function, WHO grade, AJCC staging, and lymph node metastases were associated with prognosis of pNETs(P<0.05).Conclusion:Imaging and immunohistochemistry are helpful for early diagnosis of pNETs.Compared with insulinoma, NF-pNETs have a higher degree of malignancy and a poorer prognosis.
