摘要:
研究股骨颈骨折后的不同时期股骨头软骨细胞、骨细胞凋亡以及Fas相关死亡结构域蛋白(FADD)的表达。方法:采用光镜观察、免疫组织化学和原位末端标记法对31例新鲜股骨颈骨折(新鲜骨折组)、21例陈旧股骨颈骨折(陈旧骨折组)、28例股骨颈骨折后坏死(坏死组)患者的股骨头骨细胞和软骨细胞进行组织学观察、FADD蛋白及细胞凋亡检测。结果:3组患者的标本均可见不同程度的软骨、骨细胞空陷窝化、FADD的表达及凋亡。其中,坏死组软骨细胞FADD蛋白的表达及细胞凋亡(分别为0.3176±0.0413和22±6)较新鲜骨折组和陈旧骨折组明显增强,差异有统计学意义(P<0.05),陈旧骨折组(分别为0.1338±0.0278和8±2)与新鲜骨折组(分别为0.1046±0.0215和6±2)比较差异无统计学意义。陈旧骨折组骨细胞FADD蛋白的表达及细胞凋亡(分别为0.2515±0.0379和34±7)与新鲜骨折组(分别为0.1923±0.0314和25±5)比较差异无统计学意义,与坏死组(分别为0.4584±0.0756和50±9)相比差异有统计学意义(P<0.05);新鲜骨折组骨细胞FADD蛋白的表达及细胞凋亡较坏死组明显增强,差异有统计学意义(P<0.05)。结论:FADD的表达与股骨颈骨折后软骨细胞、骨细胞的凋亡关联密切,对于股骨颈陈旧骨折如能及时正确地进行保头治疗,可以减少股骨头坏死的发生。
Abstract:
To study the relationship between FADD protein expression and chondrocyte,osteocyte apoptosis in the femoral head after femoral neck fractures.Methods:HE , immunohistochemistry ,and TUNEL were used to observe the FADD expression and apoptosis in femoral neck specimens from 77 cases of femoral neck fractures which were divided into fresh fracture,obsolete fracture and necrosis groups.Results: Different degrees of empty chondrocyte,osteocyte lacunae,the FADD expression and apoptosis appeared in all 77 cases. The FADD expression and apoptosis of chondrocyte in the necrosis group was significant higher than that in the groups of fresh fracture and obsolete fracture(P<0.05).In comparison with the group of fresh fracture,there was no significant differences in the groups of obsolete fracture; The FADD expression and apoptosis of osteocyte in the groups of obsolete fracture was a little higher than that in the groups of fresh fracture ,but was significant lower than that in the necrosis groups(P<0.05).There was a more significant difference between the necrosis groups and the groups of fresh fracture (P<0.05) .Conclusion: The expression of FADD plays an important role during the period of apoptosis of chondrocyte and osteocyte in the femoral head after femoral neck fractures. To the obsolete fractures of femoral neck,it may be possible to the declines of femoral head necrosis if prompt and correct interventions to be performed.
