摘要:
建立大鼠糖尿病性心肌病(DCM)模型,观察过氧化物酶体激活物受体α(PPARα)激动剂非诺贝特对糖尿病心肌炎症因子的作用以及心室重塑的影响。方法:采用高热量饮食联合小剂量链脲佐菌素(STZ)注射的方法诱导2型糖尿病大鼠模型。糖尿病成模后随机分为低、高剂量非诺贝特干预组和糖尿病心肌病组(DCM组),3组在糖尿病成模后均继续给予高脂高糖饲料诱发DCM模型,其中低、高剂量非诺贝特干预组分别给予4mg/kg与40mg/kg非诺贝特,另设正常对照组给予常规饲料,4组的饲养时间均为16周。16周后观察心功能、心肌超微结构,检测生化指标,胶原表达,计算心脏质量指数、胶原容积分数,免疫组织化学方法检测心肌组织中肿瘤坏死因子(TNFα)、白介素10(IL10)及PPARα的表达。结果:超声检查显示DCM大鼠心功能减退,超微结构以及病理检查显示心室重塑,其中DCM组更加明显;TNFα、IL10以及PPARα的表达也明显增加。不同剂量的非诺贝特干预后,大鼠心室重塑减轻,TNFα表达减少,IL10与PPARα表达均增加。结论:DCM时出现了明显的心室重塑,伴有心肌中炎症因子的异常表达,经过非诺贝特的干预,心室舒张功能改善,其机制可能是通过对心肌中部分炎症因子如TNFα与IL10的调节,即非诺贝特的抗炎特性实现的。
Abstract:
To observe the effects of fenofibrate on myocardial inflammation and ventricular remodeling of diabetic cardiomyopathy rats. methods: Type 2 diabetes model was established by high energy diet and low dose of STZ. The rats with type 2 diabetes were given high energy diet for 16 weeks to induce diabetic cardiomyopathy model and allocated into 3 groups,among which,2 groups were treated with high or low dose fenofibrate for 16 week, respectively, and the other group was not treated. Rats in normal control groups were only given normal diet for 16 weeks. Hemodynamic characterization was determined by echocardiography, the myocardial ultrastructure was observed by electron microscope, and the plasma biochemical indicators,heart weight index,type of Ⅰ and Ⅲ collagens, collagen volume fraction, expression of TNFα, IL10, and PPARα were detected respectively by biochemical methods, and immunohistochemistry.Results:Echocardiography and ultrastructure manifested that there were abnormal heart function and ventricular remodeling in DCM rats, especially for rats in DCM group. The expressions of ventricular remodeling, IL10, and PPARα were elevated. After the intervention of fenofibrate, the ventricular remodeling was alleviated, the expression of ventricular remodeling was decreased while that of IL10 and PPARα was increased.Conclusion: The inflammatory cytokine and remodeling might be involved in the development of diabetic cardiomyopathy;fenofibrate might play an important role in the myocardial disease via its inflammatory effects.
