摘要:
观察盐酸戊乙奎醚(PHC)对心脏瓣膜置换术患者心肌细胞NFκB活性及血浆肿瘤坏死因子α(TNFα)水平的影响,探讨其减轻心肌缺血再灌注损伤(MIRI)的可能机制。方法:选取体外循环(CPB)下行心脏瓣膜置换术的患者30例,分为对照组(C组)、PHC组1(P1组)及PHC组2(P2组),每组10例。P1、P2组在手术开始前分别给予PHC0.05mg/kg、0.1mg/kg,C组则给予生理盐水10mL。于CPB前(T1)、主动脉阻断后30min(T2)、开放后30min(T3)取右心耳组织,观察组织病理学变化,免疫组化法测定NFκB活性。于T1、T2、T3及主动脉开放后4h(T4)、24h(T5)抽取动脉血3mL,检测血浆TNFα水平,同时监测血流动力学指标。结果:P1、P2组心肌损伤程度较C组轻。C、P1和P2组患者心肌NFκB在T2、T3时均被激活,但与C组相比,P1、P2组NFκB活性下降(F组间=47.760,F时间=277.133,P<0.001);3组血浆TNFα水平在T3、T4、T5时均升高,但与C组相比,P1、P2组TNFα水平均下降(F组间=6.377,P<0.01;F时间=393.565,P<0.001)。3组血流动力学指标较稳定。结论:CPB介导的MIRI可引起心肌NFκB的激活及血浆TNFα的释放,PHC可能通过抑制NFκB及TNFα的表达从而减轻心肌损伤
Abstract:
To investigate the effect of penehyclidine hydrochloride(PHC) on the levels of myocardium nuclear factor kappa B(NFκB) and plasma tumor necrosis factoralpha(TNFα) in patients undergoing heart valve replacement with cardiopulmonary bypass(CPB), and furthermore to explore its possible mechanisms of protecting myocardium.Methods:Thirty patients undergoing valve replacement with CPB were randomly selected and divided into control group(group C),PHC group 1(group P1),PHC group 2(group P2),10 cases in each group. Before operation,PHC was injected into the central vein with the dose of 0.05 mg/kg and 0.1 mg/kg in group P1 and group P2, and 10 mL normal saline in group C, respectively. Then myocardium samples of right auricle were collected at different time points(T1: before CPB, T2:30 min after aortic clamping, T3:30 min after aortic unclamping) to observe the histological changes and the expression of NFκB.The TNFα levels in plasma were investigated at T1,T2,T3, and 4 h and 24 h after aortic unclamping(T4 and T5).Results:The myocardium injury of P1 and P2 groups were milder than that of group C.Myocardium NFκB had been activated in the three groups at T2 and T3 points, but the P1 and P2 groups were lower than group C and the differences were significant (P<0.05). The levels of TNFα of all the patients increased at T3, T4 and T5, and the levels of TNFα in P1 and P2 groups were obviously lower than that in group C(P<0.05). Conclusion:This study shows that the period of myocardial ischemia reperfusion can activate the transcription of NFκB and increase the plasma levels of TNFα in patients undergoing heart valve replacement with CPB.PHC may protect myocardium through inhibiting the transcription activity of NFκB and the expression of TNFα in the plasma.
