观察转化生长因子β1(TGFβ1)对大鼠肠黏膜缺血再灌注(I/R)损伤的保护作用。方法:30只SD大鼠随机分成3组(n=10):假手术组(A组)、I/R组(B组)和实验组(C组)。C组夹闭肠系膜上动脉(SMA)60 min,再灌注120 min,缺血前10 min经SMA注入TGFβ1溶液0.5 mL;B组以生理盐水代替TGFβ1,余同C组;A组单纯分离肠SMA,不做阻断,余同B组。采用HE染色及Chiu评分法判断小肠黏膜形态学损伤程度,检测门静脉血浆中D乳酸水平。结果:B组肠黏膜损伤较A组严重,C组肠黏膜损伤较B组明显改善。3 组间比较,Chiu评分和门静脉血D乳酸水平差异均有统计学意义(F=20.490、33.084,P均<0.001);门静脉血浆D乳酸水平与肠黏膜损伤病理评分呈正相关关系(r=0.881,P<0.001)。结论:TGFβ1可以减轻小肠I/R损伤,血D乳酸水平可以间接反映肠黏膜屏障的通透性,进而评价肠黏膜屏障的损伤程度。
To investigate protection of transforming growth factorβ1(TGFβ1) against intestinal mucosal ischemiareperfusion(I/R) injury in rats.Methods:Thirty SD rats were allocated into 3 groups (10 rats in each group) at random:shamoperation group(group A),I/R group (group B) and experimental group (group C). Superior mesenteric artery (SMA) in group C was blocked for 60 min, then reperfused for 120 min, and administered with TGFβ1 through SMA 10 min before blockage. In group B, animals underwent the same procedure as group B but taking normal saline instead of TGFβ1. In group A, SMA was only isolated but not blocked, the other procedures was the same as group B. Intestinal mucosal damage was detected by HE staining and graded according to Chiu's scoring system. Dlacate in portal vein plasma was also detected.Results:There was more destruction of intestinal villous in group B than that in group A,but the villous histology was improved in group C.The differences of Chiu's score and level of Dlacate in portal vein plasma were significant among 3 groups(F=20.490,33.084, P<0.001). There was a linear relationship between intestinal histological damage and portal vein plasma Dlactate level (r=0.881,P<0.001).Conclusion:TGFβ1 exerts a protective effect on functional and morphological injury induced by I/R, which results in an increased intestinal permeability.Plasma Dlactate level indirectly mirrors permeability of the intestinal mucosa and can be used to access the grade of intestinal mucosal damage.
