摘要:
探讨24 h间断与持续灌注低温含血停搏液对离体猪心脏冠状动脉功能的影响。方法:健康瑞典家猪18头,随机分为对照组、间断灌注组和持续灌注组3组,每组6头。间断灌注组和持续灌注组切取心脏后分别建立冠状动脉灌流模型进行24 h离体心脏间断与持续低温灌注,然后进行器官浴槽实验评价冠状动脉平滑肌和内皮细胞的功能,而对照组心脏未经灌注直接测定冠状动脉功能。结果:对照组、间断灌注组和持续灌注组的平滑肌最大收缩值(Cmax)分别为(17.33±4.07)、(17.79±3.67)和(16.89±3.61) mN,差异无统计学意义(F=0.084,P=0.919),最大舒张值(Rmax)均为100%。3组的最大内皮依赖性舒张(EDRmax)分别为(97.04±1.42)%、(91.87±2.29)%和(90.16±3.94)%(F=10.164,P=0.002),舒张至50%最大收缩时P物质浓度的负对数(pEC50)分别为(7.55±0.16)、(7.25±0.21)和(6.72±0.22)(F=26.712,P<0.001);与对照组相比,间断灌注组和持续灌注组EDRmax和pEC50均降低(P均<0.05);与间断灌注组相比,持续灌注组pEC50降低(P<0.05)。结论:24 h间断与持续低温灌注对离体心脏冠状动脉平滑肌功能无明显损伤,但对内皮细胞功能均有明显损伤,其中24 h间断低温灌注对内皮细胞功能损伤较轻。
Abstract:
To investigate the effects of longterm intermittent or continuous cold blood cardioplegia perfusion on coronary artery function of isolated porcine heart.Methods:A total of 18 Swedish domestic pigs were randomly divided into three groups: control group, intermittent perfusion group,and continuous perfusion group,with six in each group. Coronary artery perfusion models were established respectively to perfuse the isolated hearts intermittently or continuously for twentyfour hours in intermittent perfusion group or continuous perfusion group, then the function of the smooth muscle and endothelial cells of coronary artery were evaluated in organ baths. The heart without perfusion was used to evaluate the coronary artery function in control group.Results: There was no significant difference on the maximal contraction (Cmax) of smooth muscle, which was (17.33±4.07), (17.79±3.67), and (16.89±3.61)mN, respectively (F=0.084,P=0.919), and the maximal relaxation percentage (Rmax) of smooth muscle was all 100% in three groups.The maximal endothelium dependent relaxation (EDRmax) was (97.04±1.42)%, (91.87±2.29)%,and (90.16±3.94)%(F=10.164,P=0.002), and the negative logarithm of the concentration of P substance (pEC50) was (7.55±0.16),(7.25±0.21),and (6.72±0.22),respectively (F=26.712,P<0.001). Compared with control group, both EDRmax and pEC50 declined in intermittent perfusion group and continuous perfusion group (P<0.05). The pEC50 declined in continuous perfusion group in comparison with intermittent perfusion group (P<0.05).Conclusion:Although 24 h intermittent and continuous cold perfusion do not result in injury to the function of smooth muscle of coronary artery in the isolated heart, they can result in injury to the function of endothelial cells.And less injury exists in 24 h intermittent cold perfusion compared with continuous cold perfusion.
