摘要:
探讨温缺血1h对猪肺动脉内皮功能的影响,为临床“无心跳供体”(NHBD)肺应用于肺移植提供依据。方法:瑞典家猪16头,随机分为有心跳供体(HBD)组和NHBD组,每组8只,分别获取内径和长度均约1.5mm的肺动脉环,HBD组在有心跳下获取标本,NHBD组在心跳停止置于室温(20℃)1h(即温缺血1h)后获取标本,用离体组织灌流装置装置通过高钾Kreb’s溶液诱导血管环收缩,以蓄积方式加入浓度递增的Ach测定肺动脉的内皮依赖性舒张(EDR)功能和血管内皮对Ach的敏感性。结果:NHBD组由高钾(124mmol/L)Kreb’s溶液诱导的收缩反应值、肺动脉的最大EDR值和血管内皮对Ach的敏感性(用50%最大舒张时的Ach浓度即pEC50表示)分别为(25.81±1.86)mN、(96.3±2.2)%和(7.52±0.09)mol/L,与HBD组的(26.80±1.59)mN、(97.0±1.5)%和(7.37±0.05)mol/L比较,差异无统计学意义(t=1.39、0.806和1.011,P=0.274、0.434和0.329)。结论:温缺血1h的无心跳供体肺动脉内皮功能保存完好。
Abstract:
To seek the proof for the using of nonheartbeating donor(NHBD) and the new source of donor lungs by investigating the effect of onehour warm ischemia on the function of pulmonary artery endothelium,and to discuss the feasibility of the using of NHBDs.Methods:Sixteen Swedish domestic pigs were randomly divided into two groups:heartbeating donor (HBD) group and NHBD with 1 h warm ischemia group. The pulmonary artery rings whose lengths and inner diameters were both 1.5 mm were obtained from isolated left lungs of all the pigs. The pulmonary artery rings were transferred to TissueOrganBaths dividually. The Kreb’s solution with highconcentration K+ was used to cause the contraction. Acetylcholine(Ach) was used to test and compare the maximal endotheliumdependent relaxation (EDR) response of pulmonary artery and the sensitivity of pulmonary artery endothelium to Ach (expressed as the pEC50 value) between HBD and NHBD groups.Results: Between NHBD group and HBD group,maximal endothelium contraction value,EDR and the sensitivity of pulmonary artery endothelium to Ach were (25.81±1.86) mN,(96.3±2.2)%,(7.52±0.09) mol/L,and (26.80±1.59) mN,(97.0±1.5)%,(7.37±0.05) mol/L,respectively, (t=1.39,0.806 and 1.011;P=0.274,0.434 and 0.329).Conclusion: The function of pulmonary artery endothelium from NHBDs keeps intact after onehour warm ischemia.
