摘要:
目的:探讨重组纤维连接蛋白(RN)诱导的自体细胞因子诱导的杀伤细胞(CIK)细胞联合IFNα治疗晚期肝癌的可行性。方法:分离14例晚期肝癌患者外周血单个核细胞,采用RN、CD3单抗、IFNγ及IL2等细胞因子诱导产生CIK。患者每周回输一次CIK,连续4~6次,同时给予IFNα治疗。采用流式细胞术检测患者外周血淋巴细胞免疫表型,应用BDTMCBAHuman Th1/Th2型细胞因子试剂盒监测治疗前及疗程结束后7 d外周血细胞因子的变化。结果:每个患者平均回输细胞数为25.6×109 (21.6×109~31.8×109),毒副作用小。治疗后患者外周血中淋巴细胞数,CD3+,CD3+CD56+,CD3+CD8+细胞数均有明显升高。14例患者中有1例获部分缓解,4例稳定,其中缓解和2例稳定患者的AFP水平明显下降。治疗后临床获益患者血清IFNγ和TNFα水平明显升高(t=12.644和17.169,P<0.001),而治疗无反应患者细胞因子水平则无明显变化。结论:RN诱导的自体CIK联合IFNα治疗晚期肝癌是一种安全有效的过继免疫治疗方法。
Abstract:
Aim:To determine the feasibility of retronectin actived autoCIK combined with IFNα hepatocellular carsinoma(HCC).Methods:CIK were generated from peripheral blood mononuclear cells (PBMC) from 14 patients with advanced HCC induced by retronectin,CD3 monoclonal antibody,IL2,and IFNγ.CIK was transfused intravenously every week for at least 4 times and IFNα were administrated three times per week. The cytokine secretion profile was analyzed by BDTMCBA Human Th1/Th2 kit before and after treatment.Results:The median number of transferred cells per patient was 25.6×109 (21.6×109~31.8×109), and the toxicity profile were tolerable. After CIK and IFNα were administrated, the absolute median count of lymphocytes, CD3+, CD3+CD8+ and CD3+CD56+ cells significantly increased in patient’s peripheral blood. One patient had partial response (PR) and four patients had stabilization of disease(SD). The downtrend of AFP level was observed in the PR and two SD patients. In the clinical benefit patients, an increased production of IFNγ and TNFα in the serum were observed after treatment(t=12.644,17.169,P<0.001).Conclusion:Adoptive immunotherapy with retronectin induced CIK cells combined with IFNα is a safe and effective therapy.
