摘要:
目的:分析比较煤矿作业工人呼吸道诱导痰细胞学的表现。方法:对接尘非尘肺工人62名、未接尘非尘肺工人53名、尘肺患者22例和健康对照10人进行呼吸道诱痰,将痰液处理后进行细胞总数计数和分类计数。结果:接尘与未接尘非尘肺工人痰细胞总数、中性粒细胞(N)、巨噬细胞(M)、淋巴细胞(L)及嗜酸性粒细胞(Eo)计数比较,差异均无统计学意义(F分别为0.986、0.607、0.481、0.317和0.020,P均>0.05)。不同工龄接尘非尘肺工人痰细胞总数、N、L及Eo计数差异亦无统计学意义(F分别为0.431、2.390、0.341和0.351,P均>0.05),但接尘工龄10 a及以上组M计数较高(F=2.723,P=0.038)。健康对照、Ⅰ期尘肺和Ⅱ+Ⅲ期尘肺患者痰细胞总数及细胞分类计数差异无统计学意义(F分别为0.704、3.103、1.812、1.531和0.394,P均>0.05)。结论:呼吸道巨噬细胞计数可以作为尘肺进展的标志;诱导痰方法可用于粉尘作业工人痰细胞学变化的监测。
Abstract:
Aim:To analyze the changes of phlegm cytology of induced sputum from coal mine workers.Methods:A total of 62 nonpneumoconiosis workers with dust exposure,53 nonpneumoconiosis workers without dust exposure, 32 patients with pneumoconiosis,and 10 normal controls were subjected to respiratory luring, the sputum cells were totally counted and classified.Results:There were no differences in total count and classified count between nonpneumoconiosis workers with dust exposure and without dust exposure(F=0.986,0.607,0.481,0.317,and 0.020,P>0.05).There was no significant difference in the total count and classified count among different length of service of nondustexposed workers (F=0.431,2.390,0.341 and 0.351,P>0.05) and among the normal control, pneumoconiosis Ⅰ,and pneumoconiosis Ⅱ and Ⅲ patients(F=0.704,3.103,1.812,1.531,and 0.394,P>0.05).Macrophages of the workers with dust exposure more than 10 years were dramatically higher than other groups(F=2.723,P=0.038).Conclusion:The respiratory macrophages proportion may be used as a symbol for the progression of pneumoconiosis disease; induced sputum method can be applied to monitoring changes of phlegm cytology in dust homework workers.
