目的:探讨高迁移率族蛋白(HMGB)1和Toll样受体(TLR)4与子痫前期的关系。方法:选取60例子痫前期患者(其中轻度30例,重度30例)作为研究对象,以30例正常晚孕妇女作为正常对照,采用RTPCR及免疫组化法测定胎盘组织中HMGB1及TLR4 mRNA和蛋白的表达,ELISA法测定母血血浆中HMGB1和TLR4蛋白水平。结果:HMGB1主要定位于滋养细胞的胞质中,TLR4主要定位于胎盘合体滋养细胞的胞膜及胞质中。3组胎盘组织中HMGB1、TLR4 mRNA和蛋白的表达水平、母血中2种蛋白的水平差异均有统计学意义(F=79.143、35.155、6.030、10.080、12.493和11.856,P均<0.05);轻、重度子痫前期患者胎盘组织中HMGB1及TLR4 mRNA和蛋白的表达水平、母血中HMGB1和TLR4蛋白水平均高于正常晚孕妇女(P<0.05),但轻、重度子痫前期组之间差异无统计学意义(P<0.05)。子痫前期患者胎盘组织中HMGB1与TLR4 mRNA相对表达量、母血中2个蛋白的表达水平呈正相关(r分别为0.862和0.783,P<0.001),胎盘组织中2个蛋白的表达亦呈正相关(rs=0.825,P<0.001)。结论:HMGB1和TLR4表达增高与子痫前期发病密切相关,可能是子痫前期的发病机制之一。
Abstract:
Aim:To explore the relationship of HMGB1 and TLR4 with preeclampsia.Methods:Sixty patients with preeclampsia (30 cases of mild preeclampsia and 30 cases of severe preeclampsia) and 30 cases of normal term pregnancy were selected to detect HMGB1 and TLR4 mRNA and protein in placenta by semiquantitative RTPCR and immunohistochemistry,and HMGB1 and TLR4 protein in maternal peripheral blood,by ELISA.Results:HMGB1 protein was mainly expressed in trophoblast cytoplasm,TLR4 protein was mainly expressed in trophoblast membrane and cytoplasm.There were significant differences in HMGB1 and TLR4 mRNA and protein expression levels in placental tissue,and the HMGB1 and TLR4 protein expression levels in maternal blood among the 3 groups (F=79.143,35.155,6.030,10.080,12.493 and 11.856,P<0.05). The indexes mentioned above in mild and severe preeclampsia groups were higher than those in control group(P<0.05), but there was no significant difference between mild and severe preeclampsia groups(P<0.05). In preeclampsia patients,the expressions of HMGB1 mRNA and protein were positively related to those of TLR4 in placental tissue (r=0.862,rs=0.825,P<0.001),and the expression of HMGB1 protein was positively related to that of TLR4 in maternal blood(r=0.783,P<0.001). Conclusion:The high expression of HMGB1 and TLR4 may be closely related to preeclampsia, and it may be involved in the pathogenesis of preeclampsia.
