摘要:
目的:探讨更年期糖尿病女性C肽水平对骨密度的影响,为糖尿病骨量异常的治疗提供依据。方法:选择在河南省人民医院就诊或健康体检的更年期女性170例,根据C肽水平将其分为糖尿病C肽分泌减少组(A组)、糖尿病C肽分泌低下组(B组)和正常对照组(C组),行标准馒头餐试验(血糖测定应用葡萄糖氧化酶法)及C肽释放试验(化学发光法),同时采用双能X线测定正位腰椎及左侧股骨颈骨密度(BMD)。结果:糖尿病各组BMD除A组左侧股骨颈骨密度值外,其余均低于C组,其中以B组最明显(F腰椎=32.687,F左股骨颈=19.518,P<0.001);组间比较,B组C肽水平最低(F空腹=312.289,F餐后2 h=117.284, P<0.001),而相应时限血糖值最高(F空腹=383.077,F餐后2 h=418.116, P<0.001)。结论:C肽分泌功能越低相应血糖值越高,对骨密度影响越大。
Abstract:
Aim: To evaluate the therapeutic necessities of the bone mineral density(BMD) abnormality in diabetes mellitus(DM) patients, we investigated the correlation of serum Cpeptide level and BMD in adult female DM patients. Methods:A total of 170 climacteric female patients in Henan Provincial People's Hospital were divide into three groups:Cpeptide reduced group(group A),Cpeptide deficiency group(group B) and control group(group C). Blood glucose and Cpeptide level was analyzed before and after standard meal, and the BMD was measured in anterial lumber spine and in left collum femoris area. Results: The BMD in DM group was significantly lower than that in control group, this effect was more dramatic in Cpeptide deficiency group(Flumber spine=32.687,Fleft collum femoris area=19.518,P<0.001), but no difference was detected between Cpeptide reduced groups and controls in left collum femoris area. Cpeptide deficiency group had lowest Cpeptide level(Fempty stomach=312.289,Fpostprandial 2 h=117.284, P<0.001), highest blood glucose level which was statistically significant over the normal controls(Fempty stomach=383.077,Fpostprandial 2 h=418.116, P<0.001). Conclusion: Patients with lower Cpeptide have higher blood glucose level and associate with higher level of BMD abnormality.
