目的:探讨血浆同型半胱氨酸(Hcy)水平及蛋氨酸合成酶(MS)A2756G基因多态性与河南汉族人群脑梗死的关系。方法:运用循环酶法和PCRRFLP方法检测河南汉族300例脑梗死患者及261例健康对照者血浆Hcy水平及MS A2756G基因多态性。结果:河南汉族脑梗死患者血浆Hcy水平[(19.3±7.2) μmol/L]高于对照组[(15.1±4.9) μmol/L](t=8.031,P<0.001)。不同脑梗死类型间血浆Hcy水平差异无统计学意义(F=0.739,P=0.566)。Logistic回归分析结果显示:高Hcy血症是河南汉族脑梗死的独立危险因素[OR(95%CI)=1.164(1.120~1.220),P<0.001]。脑梗死组和对照组MS A2756G基因型及等位基因频率分布差异无统计学意义(χ2=0.165和0.037,P均>0.05)。不同基因型(AA、AG和GG)受检者血浆Hcy水平差异无统计学意义(F=0.110,P=0.896)。Logistic回归分析结果显示:MS A2756G基因突变与脑梗死的发生无关[OR(95%CI)=1.110(0.640~1.930),P=0.722]。结论:高Hcy血症是河南汉族脑梗死的独立危险因素;MS A2756G基因突变与Hcy水平及脑梗死的发生无明显相关。
Abstract:
Aim: To explore the relationship between the plasma homocysteine(Hcy) level,polymorphisms of methionine synthase(MS) gene and cerebral infarction in Henan Han population.Methods: Plasma Hcy level was detected with circle enzyme method and MS A2756G genotypes were detected with PCRRFLP in 300 patients with cerebral infarction and 261 controls.Results: The plasma Hcy level was significantly higher in the cerebral infarction group than that in the control group [(19.3±7.2) μmol/L vs (15.1±4.9) μmol/L] (t=8.031,P<0.001). There were no differences among stroke subtypes (F=0.739,P=0.566).The multiple stepwise logistic regression analysis showed that higher level of Hcy (HHcy) was an independent risk factor of cerebral infarction in Henan Han population [OR(95%CI)=1.164( 1.120-1.220),P<0.001].The distribution frequencies of MS A2756G genotypes and alleles were not statistically different between cerebral infarction and control groups(χ2=0.165,0.037,P>0.05). There was no significant differences in the plasma Hcy levels among 3 genotypes of MS A2756G(AA,AG,GG) (F=0.110,P=0.896).After adjusting the conventional risk factors, the multiple stepwise logistic regression analysis showed that MS A2756G gene mutations were not significantly associated with cerebral infarction occurence[OR(95%CI)=1.110(0.640-1.930),P=0.722].Conclusion: HHcy is probably an independent risk factor for cerebral infarction in Henan Han population.There is probably no relationship between MS A2756G genetic polymorphism and cerebral infarction,the same as well as the level of plasma Hcy.
