摘要:
目的:探讨血清脂联素、瘦素与2型糖尿病(T2DM)患者轻度认知功能障碍(MCI)的关系。方法:对60例T2DM患者应用蒙特利尔认知评估(MoCA)量表测评认知功能,然后分为T2DMMCI组(25例)和T2DM认知功能正常(T2DMNC)组(35例),另选择糖耐量正常且认知功能正常者25例作为对照组(NGTNC组)。对入选病例测定血清脂联素、瘦素,并测定糖化血红蛋白(HbA1c)、血脂。结果:T2DMMCI组脂联素水平为(3.31±0.22) mg/L,低于T2DMNC组的(5.39±0.38) mg/L及NGTNC组的(11.53±0.29) mg/L(F=175.921,P<0.001);T2DMMCI组瘦素水平为(16.24±0.18) μg/L,高于T2DMNC组的(13.43±0.25) μg/L及NGTNC组的(5.41±0.19) μg/L(F=475.478,P<0.001)。T2DMMCI组MoCA评分与HbA1c(r=-0.596,P=0.002)、糖尿病病程(r=-0.533,P=0.006)、瘦素(r=-0.674,P<0.001)呈负相关,与脂联素(r=0.693,P<0.001)呈正相关。多重线性回归分析结果显示:脂联素(标准化β=0.481,P=0.002)、瘦素(标准化β=-0.370,P=0.010)、HbA1c(标准化β=-0.305,P=0.012)是MoCA评分的危险因素。结论:血清脂联素、瘦素、HbA1c是T2DM患者MCI的危险因素;病程不同程度参与了T2DM患者MCI的形成。
Abstract:
Aim: To explore the relationship between mild cognitive impairment(MCI) and adiponectin and leptin in type 2 diabetes mellitus(T2DM) patients.Methods: A total of 60 T2DM patients were divided, based on cognitive function assessed by Montreal cognitive assessment(MoCA) scale, into two groups, namely 25 T2DM patients with MCI (T2DMMCI group) and 35 T2DM patients with normal cognitive function(T2DMNC group). Another 25 subjects with normal glucose tolerance and normal cognitive function were chosen as normal control(NGTNC). Serum adiponectin and leptin were measured. HbA1c and lipid levels were also determined.Results: In comparison with T2DMNC group [(5.39±0.38) mg/L] and NGTNC group [(11.53±0.29) mg/L], adiponectin was significantly lower in T2DMMCI group [(3.31±0.22) mg/L] (F=175.921, P<0.001); while in comparison with T2DMNC group [(13.43±0.25) μg/L] and NGTNC group[(5.41±0.19) μg/L], leptin was significantly higher in T2DMMCI group[(16.24±0.18) μg/L] (F=475.478, P<0.001). In T2DMMCI group, MoCA score was negatively correlated with HbA1c (r=-0.596, P=0.002), diabetic duration (r=-0.533, P=0.006) and leptin (r=-0.674, P<0.001), but was positively correlated with adiponectin (r=0.693, P<0.001). Multiple linear regression analysis suggested that adiponectin, leptin and HbA1c were the risk factors of MoCA score(standadized β=0.481,-0.370, and -0.305,P<0.05).Conclusion: Adiponectin, leptin and HbA1c are the risk factors for diabetic cognitive impairment. Diabetic duration contributes in a sense to the forming of MCI.
