摘要:
目的:研究快速解剖映射(FAM)技术在主动脉窦内起源室性早搏(简称室早)导管消融中的应用价值。方法:应用FAM技术对20例起源于主动脉窦内室早患者进行主动脉窦的解剖建模、激动标测和射频消融。观察消融即刻成功率、手术时间、消融时间和X射线曝光时间以及并发症情况。通过随访评价导管消融的远期疗效。结果:主动脉窦内室早位于左冠窦6例(30.0%),右冠窦7例(35.0%),左右冠窦交界区7例(35.0%)。放电(2.3±0.2)次室早完全终止,放电时间(66.3±5.7) s, 手术时间(1.9±0.8) h,X线曝光时间(8.4±3.5) min。所有患者术中即刻均达到消融终点,随访(10±2)个月,无一例复发。结论:采用FAM技术导管消融治疗主动脉窦内起源的室早安全、有效。
Abstract:
Aim: To investigate the value of fast anatomic mapping(FAM) technique in catheter ablation for ventricular premature contracts(VPCs) originating from the aortic sinus cusp(ASC).Methods: With FAM technique, 20 patients with VPCs originating form ASC were performed ASC anatomic reconstruction, activation mapping and ablation.The acute ablation success rate,operation time,fluoroscopy time and complication were observed.The security and efficacy of ablation were also studied by follow up.Results: The foci of VPCs were located in left ASC in 6 patients(30.0%), right ASC in 7 patients(35.0%) and the junction of left and right ASC in 7 patients(35.0%). After (2.3±0.2) firing tries,VPCs were completely abolished with discharge time of (66.3±5.7) s.The operation time and fluoroscopy time were (1.9±0.8) h and (8.4±3.5) min, respectively.All arrhythmias were successfully terminated. None of the patients had recurrence or complications during (10±2) months of followup.Conclusion: Mapping and ablation with FAM technique will improve the security and efficacy of treating VPCs originating from the ASC.
