目的:探讨弥散加权成像(DWI)在儿童髓母细胞瘤鉴别诊断中的应用价值。方法:对71例儿童幕下肿瘤行常规磁共振成像(MRI)增强检查和DWI检查,测量肿瘤实质最小表观弥散系数(ADC)。结果:髓母细胞瘤38例,平均最小ADC值为(0.51±0.11)×10-3 mm2/s; 星形细胞瘤(Ⅰ~Ⅱ级)17例,平均最小ADC值为(1.59±0.51)×10-3 mm2/s; 室管膜瘤16例,平均最小ADC值为(0.86±0.22)×10-3 mm2/s。三种肿瘤的平均最小ADC值差异有统计学意义(P<0.05)。平均最小ADC值诊断髓母细胞瘤的ROC曲线下面积为0.984±0.011。平均最小ADC值以0.626×10-3 mm2/s为阈值,与病理结果对照,诊断髓母细胞瘤的敏感度为94.7%,特异度97.0%,准确率为94.7%,与病理组织学结果行一致性检验,Kappa值为0.887。结论:常规MR检查联合DWI及ADC值测定对术前诊断儿童髓母细胞瘤有明显的优势。
Abstract:
Aim: To explore the diagnostic value of diffusion weighted MR imaging in differentiating medulloblastomas.Methods: A total of 71 children with infratentorial lesions were enrolled. All patients were performed post-contrasted and diffusion weighted MR scan by SIEMEMS 3.0T magnetic resonance system. Apparent diffusion coefficients(ADC)values of tumors were calculated.Results: Among all the cases, there were 38 cases of medulloblastomas with mean minimum ADC of(0.51±0.11)×10-3 mm2/s,17 cases of astrocytomas(Ⅰ-Ⅱ grade)with mean minimum ADC of(1.59±0.51)×10-3 mm2/s,16 cases of ependymomas with mean minimum ADC of(0.86±0.22)×10-3 mm2/s.Differences of mean minimum ADC values among these tumors were statistically significant(P<0.05). The area under the ROC curve of mean minimum ADC diagnosing medulloblastoma was 0.984±0.011. An ADC of less than 0.626×10-3 mm2/s was optimal for predicting medulloblastoma with sensitivity of 94.7%,specificity of 97.0%,and accuracy of 94.7%. Compared with pathological results, mean minimum ADC had high correlations(Kappa value was 0.887).Conclusion: Diffusion-weighted imaging can be applied as a complementary tool in the detection of medulloblastomas of children.
