摘要:
目的:研究成人胫骨近端骨缺损的生物力学特征,为临床是否选用内固定提供依据。方法:选择21具新鲜成年人下肢标本,其中男性12具,女性9具。将42只胫骨标本制成胫骨平台负重区骨缺损模型,随机分为7组,分别为A1、A2、A3、B1、B2、B3和C组,每组6只。A1、A2、A3组制成胫骨近端内侧圆形骨缺损,直径分别为10、15、20 mm。B1、B2、B3组制成胫骨近端外侧圆形骨缺损,直径分别为10、15、20 mm。C组胫骨标本不作处理,为正常对照组。分别对其进行生物力学测试,行轴向压缩、三点弯曲及扭转力学实验。结果:A1、A2、B1、B2、C组胫骨平台均无塌陷,A3、B3组分别出现1例胫骨平台塌陷。各组中胫骨标本物理特性的比较差异无统计学意义(P>0.05)。A2、A3和B3组的轴向压缩刚度、三点弯曲刚度及扭转刚度较C组降低(P<0.05),A3组的轴向压缩刚度、三点弯曲刚度及扭转刚度较A2组降低(P<0.05)。结论:成人胫骨近端内侧骨皮质缺损≥15 mm或外侧骨皮质缺损≥20 mm时生物力学性能降低,临床治疗其骨缺损时需要辅助固定材料。
Abstract:
Aim: To study the biomechanical characteristics of different part and size of the adult proximal tibia bone defect,and to provide the basis for clinical practice.Methods: A total of 21 fresh adult cadaver tibial specimens were selected,including 12 males and 9 females. Forty-two tibial specimens made from tibia platform weight-bearing area bone defect model were randomly divided into 7 groups:group A1,A2,A3,B1,B2,B3 and group C.6 specimens in each group. group A1, A2, and A3 were made of proximal medial tibial bone defect with diameter of 10, 15, and 20 mm. Group B1, B2, and B3 were made of proximal lateral tibial bone defect with diameter of 10, 15, 20 mm. Tibial specimens from group C were not treated as normal control group.Axial compression test, three point bending test and torsion test were performed.Results: The tibial plateau of group A1, A2, B1, B2, and C were not collapsed, and the tibial plateau in one specimen of group A3, and B3 was collapsed,respectively. There were no significant differences in the physical characteristics of the tibial specimens among the groups(P>0.05). Compared with group C,the axial compression stiffness, three point bending stiffness and torsion stiffness of group A2,A3 and B3 decreased(P<0.05).Compared with group A2,the axial compression stiffness, three point bending stiffness and torsion stiffness of group A3 decreased(P<0.05).Conclusion: Clinical treatment of bone defects need auxiliary fixed materials,when proximal tibia of adult medial cortical defect is ≥15 mm or lateral cortical bone defect is ≥20 mm.
