摘要:
目的:探讨盐酸氨基葡萄糖对膝骨关节炎膝关节软骨下骨Ⅰ型胶原及骨钙素表达的影响。方法:选取46例因膝关节骨性关节炎行人工膝关节表面置换术的绝经后女性患者,其中23例术前半年内行盐酸氨基葡萄糖治疗12周,为观察组,未服用者23例为对照组。术中留取2组膝关节胫骨平台截骨后软骨下骨组织0.5 cm3,分别行骨组织显微CT检测、Ⅰ型胶原胶原苦味酸-天狼星红染色、骨钙素免疫组化染色、Ⅰ型胶原及骨钙素Western blot检测。结果:经显微CT检测发现,观察组软骨下骨小梁相对骨体积、骨小梁间隙、骨小梁厚度、骨小梁数目均较对照组有所改善,且观察组Ⅰ型胶原和骨钙素含量均较对照组增加(P<0.05)。Western blot检测亦证实观察组软骨下骨Ⅰ型胶原及骨钙素表达较对照组增多(P<0.05)。结论:盐酸氨基葡萄糖可能通过调节Ⅰ型胶原及骨钙素的表达,从而改善女性膝关节骨性关节炎绝经患者膝关节软骨下骨的微结构,进而改善膝关节软骨下骨功能。
Abstract:
Aim: To research the effect of glucosamine hydrochloride on type Ⅰcollagen and osteocalcin level in knee subchondral bone of patients with knee osteoarthritis.Methods: Forty-six postmenopausal female patients with knee osteoarthritis underwent TKA surgery were allocated into 2 groups, those accepted glucosamine hydrochloride oral administration for 12 weeks during the half year before surgery were observation group, and those without the glucosamine hydrochloride treatment were control group. The subchondral bone tissue(0.5 cm3)of tibial in the 2 groups was collected and detected by micro-CT, collagen picric acid sirius red staining, osteocalcin immunohistochemical staining, as well as collagen Ⅰ and osteocalcin Western blot detection.Results: Compared with control group, the relative bone volume, intertrabecular spaces, trabecular thickness and numbers of trabecular all improved in the observation group. According to collagen picric acid sirius red staining and immunohistochemical staining, the contents of type Ⅰ collagen and osteocalcin in the observation group were higher than those in control group(P<0.05). According to the results of Western blot,the expression levels of type Ⅰ collagen and osteocalcin in the observation group were higher than those in control group(P<0.05).Conclusion: Glucosamine hydrochloride can effectively improve the subchondral bone microstructure and function of knee in female patients with knee osteoarthritis by regulating the expressions of type Ⅰ collagen and osteocalcin in subchondral bone tissue.
