摘要:
目的:检测肺癌患者血清中抑癌基因p16、FHIT、APC的甲基化,探讨它们在肺癌发生和诊断中的临床价值。方法:收集120例原发性肺癌患者(肺癌组)、46例肺良性疾病患者(对照组)的血液标本,采用甲基化PCR方法检测2组患者血清p16、FHIT、APC基因甲基化。采用logistic回归分析肺癌发生的危险因素,计算上述基因甲基化诊断肺癌的准确率。结果:肺癌组血清中p16、FHIT、APC的甲基化率分别为37.5%、34.2%、31.7%,对照组中分别为6.5%、13.0%、10.9%,差异有统计学意义(P均<0.05)。Logistic回归分析提示p16、FHIT基因甲基化均为肺癌发生的独立危险因素,OR(95%CI)分别为7.509(2.160~26.099)、2.927(1.096~7.814)(P均<0.05)。p16、FHIT、APC基因甲基化单项诊断肺癌的准确率分别为53%、49%、47%,3种基因甲基化联合诊断肺癌的准确率为87%,优于单项指标。结论:p16、FHIT基因甲基化均为肺癌发生的独立危险因素; 联合检测p16、FHIT、APC甲基化可提高肺癌诊断的准确性。
Abstract:
Aim: To study the roles of p16, FHIT and APC methylation in carcinogenesis and diagnosis of lung cancer.Methods: Methylation PCR was used to detect the methylation of p16,FHIT,and APC genes in 120 cases of lung cancer(lung cancer group)and 46 cases of benign lung disease(control group). The risk factors for lung cancer were researched by logistic multivariate analysis, and the accuracy of gene methylation in the diagnosis of lung cancer was assessed.Results: The rates of p16, FHIT, and APC gene methylation in lung cancer group were 37.5%,34.2% and 31.7%, those in control group were 6.5%,13.0%, and 10.9%, and the differences were significant(P<0.05). Logistic multivariate analysis suggested that p16 and FHIT gene methylation was risk factor for lung cancer(P<0.05)[OR(95%CI)=7.509(2.160-26.099),2.927(1.096-7.814)].The accuracy of p16,FHIT,APC gene methylation single use for diagnosis of lung cancer was 53%,49%,47%, while the accuracy of the 3 gene combined detection was 87%, better than single detection.Conclusion: The methylation of p16 and FHIT gene may be related to the carcinogenesis of lung cancer,and the com bined detection may improve the accuracy of diagnosis of lung cancer.
