摘要:
目的:系统评价达芬奇机器人与腹腔镜两种术式在早期卵巢癌手术中的安全性与近期疗效。方法:检索PubMed、EMBASE、CBM、中国知网、万方、维普数据库,收集自建库以来至2017年12月各数据库有关早期卵巢癌不同术式的随机对照研究、队列研究或病例对照研究,利用RevMan 5.3进行meta分析。结果:经过严格文献质量评价,最终7篇文献共282例病例纳入本研究,其中达芬奇机器人辅助早期卵巢癌手术151例,腹腔镜辅助早期卵巢癌手术131例。Meta分析结果显示,与腹腔镜手术治疗早期卵巢癌相比,达芬奇机器人辅助早期卵巢癌手术具有以下特点,术后住院时间短(SMD=-0.93,95%CI=-1.48~-0.37,P=0.001),输血率低(OR=0.01,95%CI=0.01~0.88,P=0.04),总淋巴结切除数目多(SMD=4.73,95%CI=3.06~5.41,P<0.001),而手术时间(SMD=0.34,95%CI=-36.56~37.24,P=0.99)、术中失血量(SMD=-75.63,95%CI=-179.46~28.21,P=0.15)、术后排气时间(SMD=-0.16,95%CI=-0.46~0.13,P=0.27)、术中并发症(OR=0.85,95%CI=0.19~3.90,P=0.84)、术后并发症(OR=0.84,95%CI=0.28~2.46,P=0.74)比较,差异无统计学意义。结论:与传统的微创手术相比,达芬奇机器人在早期卵巢癌手术中具有总淋巴结切除数多、患者住院时间短及输血率低等优势。
Abstract:
Aim:To systematically evaluate the safety and short-term efficacy of Da Vinci robot and laparoscopy in the treatment of early ovarian cancer.Methods:The databases PubMed, EMBASE,CBM, CNKI, Wanfang and VIP were searched.Randomized controlled studies, cohort studies and case-control studies on different forms of early ovarian cancer were collected from the date of self-construction to December 2017. Software Revman 5.3 was used for meta analysis.Results: After rigorous literacy evaluation, a total of 282 cases were included in the study in the last seven articles, including 151 cases of Da Vinci robotic assisted ovarian cancer surgery and 131 cases of laparoscopic assisted ovarian cancer surgery. Meta analysis showed that compared with laparoscopic surgery for early ovarian cancer, the robot had shorter postoperative hospital stay(SMD=-0.93, 95%CI=-1.48--0.37, P=0.001), less intraoperative blood transfusion(OR=0.01, 95%CI=0.01-0.88, P=0.04),more lymph nodes resection(SMD=4.73, 95%CI=3.06-5.41, P<0.001). The differences between operation time(SMD=0.34,95%CI=-36.56-37.24,P=0.99), blood loss(SMD=-75.63,95%CI=-179.46-28.21,P=0.15), postoperative intestinal exhaust time(SMD=-0.16,95%CI=-0.46-0.13,P=0.27), intraoperative complications(OR=0.85,95%CI=0.19-3.90,P=0.84)and postoperative complications(OR= 0.84, 95%CI=0.28-2.46, P=0.74)were not significant.Conclusion:Compared with traditional laparoscopic surgery, Da Vinci robot has the advantages of more total resected lymph nodes, shorter hospital stay,and less blood transfusion in early ovarian cancer surgery.
