摘要:
目的:研究CREB1-rs11904814、BDNF-rs6265多态性与河南汉族老年男性卒中后抑郁(PSD)的关系。方法:以319例首次发生脑卒中的河南汉族老年男性患者为研究对象,根据HAMD 17项评分将患者分为PSD组(142例)、卒中对照组(177例); 根据PSQI评分又将PSD患者分为伴睡眠障碍组(103例)和不伴睡眠障碍组(39例)。应用PCR-RFLP技术及基因组测序技术检测患者CREB1-rs11904814和BDNF-rs6265位点多态性。结果:PSD组BDNF-rs6265 AA基因型及A等位基因频率大于卒中对照组(P<0.05),A等位基因与PSD有关联(OR=1.882,95%CI=1.344~2.635)。CREB1-rs11904814基因型及等位基因频率在PSD组和卒中对照组之间的分布差异无统计学意义(P>0.05); 但CREB1-rs11904814 G等位基因频率在PSD伴睡眠障碍组和不伴睡眠障碍组之间的分布存在差异(P<0.05),G等位基因与PSD伴睡眠障碍有关联(OR=2.729,95%CI=1.456~5.115)。结论:BDNF-rs6265 A等位基因可能是河南汉族老年男性PSD的危险因素; CREB1-rs11904814可能与河南汉族老年男性PSD发病无关,但可能是PSD合并睡眠障碍的预测因素。
Abstract:
Aim:To investigate whether the rs11904814 single nucleotide polymorphism(SNP)of cyclic adenosine monophosphate response element-binding protein 1(CREB1)gene and the rs6265 SNP of brain-derived neurotrophic factor(BDNF)gene were associated with post-stroke depression(PSD).Methods:A total of 319 stroke patients that were onset for the first time were adopted and classified as PSD(n=142)and control(n=177)groups according to HAMD(17 items)scores, and then the PSD group was further divided into PSD with sleep disturbance(n=103)and PSD without sleep disturbance(n=39)groups according to PSQI.PCR-RFLP and genome sequencing analyses were used to identify the genotypes of CREB1-rs11904814 and BDNF-rs6265.Results:The frequencies of AA genotype and A allele in the PSD group were significantly higher than those in the control group(P<0.05). A allele was positively associated with PSD in older males(OR=1.882, 95%CI=1.344-2.635). CREB1-rs11904814 genotypes and alleles showed no significant differences between the PSD group and the control group(P>0.05), but the frequency of G allele in the PSD with sleep disturbance group and the PSD without sleep disturbance group(P<0.05),and there was a significant correlation between G allele carriers and sleep disturbance(OR=2.729, 95%CI=1.456-5.115).Conclusion:BDNF-rs6265 A allele maybe a susceptibility factor of PSD. CREB1-rs11904814 SNP may not correlate with PSD, but may be a predictive factor for the sleep disturbance of PSD patients.
