摘要:
目的:探讨郑州市二七区孕母产前相关因素与围产儿先天性心脏病(CHD)间的关联。方法:收集整理2012至2016年郑州市二七区以医院为基础出生缺陷的监测数据。首先采用倾向得分匹配(PSM)方法,以围产儿性别、身长、出生体重,孕母胎盘、胎位、胎龄、羊水性状、产后出血作协变量,对CHD患儿和无出生缺陷围产儿作1:4匹配,产生均衡协变量的病例-对照数据集; 再以年龄、文化程度、孕次等6个孕母分娩前相关因素为研究变量,采用条件logistic回归作影响因素分析,并对有意义变量作亚组分析、中介效应分析。结果:共收集围产儿监测数据78 292例,其中CHD患儿1 537人。PSM匹配对照组6 148例。条件logistic回归结果示:孕母年龄、孕次、文化程度、孕产保健与围产儿CHD有关联[OR(95%CI)分别为:1.013(1.002~1.024)、1.199(1.032~1.414)、0.653(0.589~0.723)、0.629(0.432~0.914)]。孕母有孕产保健、孕次1次、产次1次和年龄25~44岁各亚人群中孕母文化程度与CHD有关联(P均<0.05); 孕产保健在文化程度与CHD间无中介效应。结论:孕母年龄大、孕次≥3次、文化程度低及无孕产保健是围产儿CHD的危险因素,应予以重视。
Abstract:
Aim:To investigate the relationship between maternal prenatal factors and the susceptibility of perinatal congenital heart disease(CHD)in Zhengzhou Erqi District.Methods:The surveillance data of birth defects from Erqi District,Zhengzhou City from 2012 to 2016 were collected. Propensity score matching(PSM)was used to generate the 1:4 case-control data set after equalizing 8 covariates,including prenatal gender,body length, birth weight,and maternal placenta,fetal position,amniotic fluid,postpartum hemorrhage.Then,conditional logistic regression was used to analyze the relationship between 6 maternal prenatal variables,including maternal age at delivery,educational level,pregnancy times and so on.Subgroup analysis and mediating effect analysis were performed for several special variables.Results:A total of 78 292 perinatal surveillance data were collected, among which 1 537 were the perinatal infants with CHD; 6 148 controls were matched by PSM.Conditional logistic regression showed that maternal age,the number of pregnancies,educational level,gestational health care were associated with CHD,OR(95%CI)were 1.013(1.002-1.024),1.199(1.032-1.414),0.653(0.589-0.723)and 0.629(0.432-0.914), respectively.The relationship between maternal educational level and the risk of perinatal CHD were still obvious in those sub-populations of mothers with maternal health care,pregnancy order 1,birth order 1,and ages of 25~44 years(all P<0.05).But the mediating effect of gestational health care in relationship between educational level and CHD was not detected.Conclusion:Advanced maternal age,pregnancy times≥3,lower educational level and with no gestational health care can increase the susceptibility of perinatal CHD,which should be controlled in prenatal care.
