摘要:
目的:分析传统树脂材料的窝沟封闭技术和借鉴非创伤性修复(ART)技术的窝沟封闭技术预防小学生恒磨牙窝沟龋的成本效果情况。方法:329名小学生纳入研究并随机分为两组,组1(126人,453颗第一恒磨牙)接受应用传统树脂材料的窝沟封闭,组2(203人,675颗第一恒磨牙)接受ART技术的窝沟封闭。18个月后随访,记录接受封闭的恒磨牙的状态以及封闭剂的保留情况。比较两种方法预防年轻恒磨牙窝沟龋的效果,选取最小成本分析比较预防成本,应用单因素敏感分析评价不同因素对最小成本的影响。结果:治疗前两组小学生在性别、刷牙频率、吃甜食频率、就诊经历及第一恒磨牙情况等方面差异均无统计学意义(P>0.05),但在年龄和含氟牙膏知晓情况方面差异有统计学意义(P<0.05)。18个月时两组封闭剂保留情况和患龋率差异均无统计学意义(P>0.05); 经logistic回归分析校正基线时不匹配因素后,两组的患龋率差异亦无统计学意义(P>0.05)。最小成本分析结果显示,组1、组2封闭一颗恒磨牙的成本分别为26.17元和22.38元。蒙特卡罗模拟(1 000次)结果显示,组1总成本高于组2的概率为79.6%。结论:综合考虑预防效果和成本,ART窝沟封闭技术更适合在预防小学生恒磨牙窝沟龋的公共卫生项目中进行推广和应用。
Abstract:
Aim:To investigate the costs and effects of two sealing methods(using resin sealant or glass ionomer material with the aid of ART technique)in preventing pit and fissure caries in permanent molars of primary school students.Methods:A total of 329 primary school students were randomly allocated into group 1(resin sealant was provided, 126 children, 453 molars)and group 2(ART sealant was provided, 203 children, 675 molars), the costs and effects of group 1 and group 2 in preventing pit and fissure caries in permanent first molars after 18 months were evaluated. Based on similar effectiveness results of the two groups, cost-minimization would be chosen for preventive cost comparison, univariate sensitivity analysis was conducted to determine affecting factors on final cost.Results:There was no significant differences between the two groups of students in gender distribution, tooth brushing habit, candy snacking habit, dental treatment experience or the status of the first permanent molars(P>0.05). Significant differences existed in their age distribution and fluoride toothpaste awareness(P<0.05). After 18 months, there was no significant difference between the two groups in sealant remaining or dental caries occurrence(P>0.05). Logistic regression analysis showed that there was no significant difference in dental caries occurrence rate between the two groups(P>0.05). Cost-minimization analysis showed that the average cost of sealing one tooth was ¥26.17 in group 1 and ¥22.38 in group 2. Monte Carlo simulations(1 000 times)indicated that the probability of the cost of group 1 being higher than that of group 2 was 79.6%.Conclusion:Considering the cost and effectiveness performance, ART sealant would be more appropriate than resin sealant in public health project for preventing fissure caries in permanent molars of primary school students.
