摘要:
目的:探讨非酒精性脂肪肝(NAFLD)的影响因素及转氨酶水平与NAFLD的关系。方法: 选取健康体检者3 488人,其中NAFLD患者1 469例,无NAFLD者2 019例,收集一般资料及体格检查、生化指标检测、腹部超声检查的资料,分析NAFLD发生的影响因素。结果:ALT和AST处于正常范围内中位数以上的被检者NAFLD患病率显著高于二者处于中位数及以下者(P<0.001)。NAFLD和无NAFLD者性别、年龄、BMI、谷氨酰转肽酶(GGT)、碱性磷酸酶(ALT)、胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、空腹血糖(FPG)的差异有统计学意义(P均<0.001)。Logistic回归分析显示男性(OR=0.751, 95%CI=0.708~0.797)、高BMI(OR=5.738, 95%CI=5.412~6.084)、高FPG(OR=2.252, 95%CI=2.063~2.459)、高TG(OR=3.036, 95%CI=2.851~3.232)、高ALT(OR=2.300, 95%CI=2.171~2.437)、高GGT(OR=2.309, 95%CI=2.047~2.605)是NAFLD的影响因素。结论:转氨酶水平升高对诊断NAFLD有重要价值,早期采取合理的降低转氨酶水平的措施有可能逆转脂肪肝的发生。
Abstract:
Aim:To investigate the risk factors of non-alcoholic fatty liver disease(NAFLD)and the relationship between transaminase level and NAFLD.Methods:A total of 3 488 patients in the physical examination center were included in this study. Physical examination data, laboratory assessments as well as hepatic ultrasound examination results were collected,and the risk factors of NAFLD were analyzed.Results:There was a significant difference in the NAFLD between the ALT and AST of normal high value and low value. The NAFLD group favored male and older,had higher BMI, higher GGT, higher ALP, higher TC,higher TG, lower HDL, higher LDL,higher FPG(P<0.001). Based on logistic regression,male(OR=0.751, 95%CI=0.708-0.797),higher BMI(OR=5.738, 95%CI=5.412-6.084),higher FPG(OR=2.252, 95%CI=2.063-2.459),higher TG(OR=3.036, 95%CI=2.851-3.232),higher ALT(OR=2.300, 95%CI=2.171-2.437),higher GGT(OR=2.309, 95%CI=2.047-2.605)were risk factors of NAFLD.Conclusions:Higher transaminase value is valuable for early diagnosing of NAFLD,which is helpful to prevent the development of NAFLD.
