摘要:
目的:分析三维超声对子宫内膜癌诊断及术前分期的应用价值。方法:收集106例高危疑诊子宫内膜癌患者的临床资料。患者术前均完成三维超声检查,进行分期与血流分级。结果:术后病理证实106例中,子宫内膜癌73例(子宫内膜样腺癌71例,浆液性乳头状腺癌2例),良性病变33例。73例FIGO分期Ⅰa期11例,Ⅰb期21例,Ⅰc期14例,Ⅱ期8例,Ⅲ期12例,Ⅳ期7例。以术后病理结果为金标准,三维超声诊断子宫内膜癌的灵敏度、特异度分别为98.63%、81.82%; 分期结果符合率为95.89%。超声分期与血流分级呈正相关(rs=0.730,P<0.001)。结论:三维超声对子宫内膜癌诊断及术前分期与病理符合度较高,可用于筛查和指导术前分期。
Abstract:
Aim: To analyze the application value of 3D ultrasound in the diagnosis and preoperative staging of endometrial cancer.Methods:The clinical data of 106 patients with high risk of endometrial cancer were collected.All patients underwent 3D ultrasonography before the operation,and underwent staging and blood flow grading.Results:Out of the 106 patients,73 were pathologically confirmed endometrial cancer(71 with endometrioid adenocarcinoma,2 with serous papillary adenocarcinoma)and 33 were benign.There were 11 cases of stage Ⅰa,21 cases of stage Ⅰb,14 cases of stage Ⅰc,8 cases of stage Ⅱ,12 cases of stage Ⅲ and 7 cases of stage Ⅳ.The sensitivity and specificity of 3D ultrasound were 98.63% and 81.82%,respectively,and the coincidence rate of staging was 95.89%,according to the gold standard of postoperative pathology.Ultrasound staging was positively correlated with blood flow grading(rs=0.730,P<0.001).Conclusion:3D ultrasound has a high concordance with endometrial cancer diagnosis and preoperative staging,and can be used for screening and guiding preoperative staging.
